Personalized Functional Medicine Memberships for Whole-Person Care

A Functional Medicine Membership provides access to a relationship-based primary care model that may include comprehensive consultations, preventive care support, annual physical exams, functional and alternative medicine discussions, and unlimited portal messaging. Members can also use in-home or virtual visit options when appropriate. Specific recommendations are individualized and based on your health history, goals, and care needs.

The membership is a self-pay arrangement and does not function as health insurance. Holistic Direct Primary Care does not accept insurance, Medicare, or Medicaid, and members do not submit insurance claims through the practice. The membership supports primary and whole-person care, while patients should maintain insurance or other financial planning for emergencies, hospital care, specialists, medications, and services outside the practice.

Membership pricing is based on age: $80 for ages 18–25, $130 for ages 26–55, and $155 for ages 56 and older. There are no enrollment or registration fees. A seasonal Snowbird Membership for adults 56 and older is $620 and includes five in-home visits per season. Non-member visits are $210.

Holistic Direct Primary Care offers same-day or next-day appointment availability when scheduling permits. Timing depends on the type of visit, the provider’s calendar, and whether an in-home or virtual format is appropriate. Members can also use unlimited portal messaging for ongoing communication, questions, and care coordination between scheduled appointments.

In-home services are available in Bradenton, Manatee County, Sarasota County, and approximately 25 miles surrounding Bradenton, Florida. Virtual visits may offer an additional convenient option when appropriate.
